We also have some New Patient paperwork you can fill out ahead of time, to save time at your first appointment. You can either fill it out (using Adobe Fill & Sign or something similar) and email it to us at office@jteravestdds.com .

We can file claims with most major dental insurances- Although we are Out-of-Network with all plans. Call our office for more information!
